Appointment Pass - The school day appointment pass should be completed and turned into the office upon return. Students missing school for a medical, dental, optical, chiropractic, or other appointments will need to complete this form in order for the missed day to be declared and excused absence.
Free and Reduced Lunch Application -The free and reduced lunch application may be filled out electronically, printed, and turned in during registration. You may also complete the form electronically and submit it electronically through Family Access. To do so, log on to Family Access and select Verify Student Information on the left and then Add a Food Service Application to the right.
New Student Registration Form -This form must be completed by all middle and high school students who have not attended Fennimore Schools in the past. In addition, please call the Fennimore Middle & High School at 608-822-3245 to communicate your intent to enroll before you complete and send forms.

Internet Acceptable Use Form -Middle and High School students must have this form signed and turned in to the Middle & High School Office to be able to use the Internet at Fennimore Schools.
The following forms are required to be on file for all athletes. Student Athletes are encouraged to print and complete each form. Return the required documents to your coach or the high school office.
